Community Nurse - Band 5

Location: Carshalton, Greater London

Pay: £21 per hour

Start- ASAP

Schedule - Monday - Friday (9:00 - 17:00) or 07:00 - 19:00 (3-4 days)

Are you a dedicated and experienced Community Nurse seeking a professional opportunity to contribute to the enhancement of community healthcare in Carshalton?

An exciting position is available for a Band 5 Community Nurse, where your expertise and commitment to patient-centered care will be highly valued.

Key Responsibilities:

* Conduct comprehensive assessments and develop individualized care plans for patients in the community setting.

* Administer skilled nursing care, including wound care and medication management, while maintaining a high standard of clinical excellence.

* Collaborate with multidisciplinary teams, fostering effective communication with general practitioners, social workers, and other healthcare professionals to ensure seamless and coordinated patient care.

* Support patients in managing chronic conditions, promoting health education, and facilitating self-care.

* Actively participate in health promotion initiatives and community outreach programs.

Requirements:

* Registered Nurse with an active NMC registration.

* Previous experience in community nursing is advantageous.

* Strong interpersonal and communication skills, with the ability to work both independently and collaboratively.

* Proficient in utilizing electronic health records and other healthcare technologies.
