Main duties of the job
* Delivering comprehensive nursing care to patients in their homes, covering wound care, medication administration, and chronic disease management
* Assessing patient needs and crafting personalized care plans tailored to their unique circumstances.
* Empowering patients and their families with knowledge and strategies to manage their health effectively.
* Collaborating seamlessly with other healthcare professionals, including GPs, social workers, and occupational therapists
* Contributing to the development and implementation of innovative community nursing initiatives
* Keeping patient records organized and up to date to ensure seamless care coordination
To excel in this role, you'll bring:
* Registered Nurse (RN) status with valid Nursing and Midwifery Council (NMC) registration
* Proven experience as a Community Nurse or in a relevant clinical setting
* Exceptional communication and interpersonal skills to build trust and rapport with patients.
* The versatility to work independently and collaboratively as part of a team.
* Impeccable organizational and time management skills to navigate a busy schedule.
* A valid driving license for convenient patient visits
